What is a Client Intake Form?

CLIENT INTAKE FORM (APPLICATION)

A Client Intake Form (or Client Application) is a required form (to be completed by prospective client) that we use to gather information about you and your business. Completing the form helps us to align expectations, budget, timeline, determine fit, provide a custom quote for services, develop a statement of work (or contract) with tangible agreed upon deliverables, and agree on payment terms / schedule. A Client Intake Form (application) must be completed prior to booking of a Call (ex: Discovery Call, Talent Strategy call, and Consulting & Coaching Session. You only need to complete the application one time provided there have not been major changes (revenue, budget, number of employees etc.) since your most recent application.

  • Please note that we do not accept projects or clients that we do not believe will be a mutual fit. We will only enter an agreement if we are confident in our ability to provide the requested service, if budget/timeline/ expectations align, and where we believe an amicable client-consultant rapport can be established. We handle our clients with care and expect respect, communication, and commitment to your goals as well.

  • We do not enter agreements with anyone we believe is not ready to perform the work required to drive the desired result or whose values are outside of alignment with our people centric and high integrity philosophy.
